Sports Physical

Dear Parents and Guardians,

Participation in sports is a great way to increase physical activity, learn how to be part of a team and be more involved at school. If your child is planning to participate in sports, he or she must have a sports physical on file. A sports physical must be submitted prior to participating in any sports tryouts.

Physicals must be documented on the school form which can be obtained from the school district’s athletic website www.gobrewerwitches.com. You will need to register your child for each sports season on that website. Copies of the physical form are also available in student Services at BCS.

One physical exam is sufficient for all of 6th, 7th and 8th grade sports participation. Physicals done prior to January of your son’s or daughter’s 6th grade year will not be accepted. (For example, if your child is entering 6th grade in the fall of 2020, a physical done in January of 2020 or after will be accepted.) If your child has annual physicals, an updated sports physical form is always welcome! 

If your child is trying out for cross country in 5th grade, a sports physical is required. Please note that an additional sports physical must be submitted the next year for 6th, 7th and 8th grade as noted above. 

Physicals need to be on file prior to sports tryouts in the fall. Please schedule an appointment with your child’s physician for a physical prior to the start of the sports season. Children who are enrolled in the School Based Health Center may have a sports physical done at school.
